Firvanq (Vancomycin Hydrochloride)
- Each kit of Firvanq consists of vancomycin hydrochloride USP, powder for oral solution, equivalent to 3.75 grams or 7.5 grams or 15.0 grams vancomycin, and grape-flavored diluent. Clostridium difficile-associated diarrhea: Adults (18 years and older): 125 mg by mouth four times daily for 10 days, Pediatrics (less than 18 years old): 40 mg per kg by mouth in 3 or 4 divided doses for 7 to 10 days. Do not exceed 2,000 mg daily. Enterocolitis caused byStaphylococcus aureus: Adults (18 years and older): 500 mg to 2,000 mg by mouth in 3 or 4 divided doses for 7 to 10 days., Pediatrics (less than 18 years old): 40 mg per kg by mouth in 3 or 4 divided doses for 7 to 10 days. Do not exceed 2,000 mg daily. Shake Firvanq oral solution well before each dose. Use a measuring device to measure out the appropriate dose (in milliliters). Store Firvanq in the refrigerator. Discard Firvanq after 14 days or if it looks cloudy or has particulates.
